Camp Policies- please read and check off the following. *

By checking each box you agree to the following policies.

I hereby enroll my child in the Camp Gan Izzy Summer Camp at the Jewish Early Childhood Center. I understand and agree to the terms stated on the above form. I hereby agree to pay the full amount due for those minimum enrollment terms.

In the event of an emergency, if I am unable to be contacted, I hereby grant permission to Camp Gan Izzy to authorize any emergency action necessary to insure the safety of my child.

Participation in any Camp Gan Izzy activities and use of facilities involves a risk of accidental injury despite all safety precautions. Having been informed of the activities conducted at Camp Gan Izzy, I, as an individual or as a parent or guardian of the participant named herein, assume all risks and hazard incidental to the activities and release from responsibility an agree to indemnify and hold harmless Camp Gan Izzy, its officers, directors, independent contractors, volunteers and all employees for any illness or injury to me or my child(ren) or family members occurring during his/her participation in any activities or use of any recreational facilities at/or conducted by Camp Gan Izzy.

I agree to sign and return all required forms with payment before the start of the program.

In order to better serve your child(ren), it may be helpful for us to contact his/her previous preschool.

Children's medical records must be signed and received via Brightwheel by May 1, 2025.

$25 off registration fee for each child if two or more children within the same family enroll at Camp Gan Izzy.

I understand that the $200 registration fee is non-refundable.

If registering after March 1, 2025, the first camp payment must be paid together with the $200 application fee.

All camp fees must be paid in full at the time of registration if received after May 1, 2025.

I am responsible for the entire summer tuition regardless of vacation, absences or removal from the program.
